Pay for one appointment at a time.
We bill in network with Medicare, Tricare for Life, Tricare Select and some auto insurances. For all private insurance we are out of network. We will provide you with the documentation you need to submit for reimbursement from your insurance company.

This will allow you to keep your body moving and performing at your highest level for the long-term, in order to continue to participate in the activities you enjoy.
Monthly membership to the clinic which includes a set number of one on one Physical Therapy visits per month, as recommended by your provider, as well as access to the gym area during clinic hours.
Benefits include reduced cost of one-hour appointments scheduled in advance, preference is given to membership plan members before new intake or a-la-cart schedulers.
Guidelines:
Membership cost will be billed monthly, not at the time of service. A three month minimum is required to participate and then can be discontinued at any time. One re-scheduled appointment will be allowed. A minimum of 24 hours advance notice is required. Please note, it is up to you to schedule your appointment in advance. Scheduling is available via an online link or by calling the clinic (904) 734-0200. If you get to the end of your monthly cycle and do not give adequate time for the appointment to be scheduled, you will be offered a waitlist appointment time. Visits will not be pushed into the next month due to improper planning.
Additional visits if needed will be charged at a self-pay rate associated with your member level, cost per visit. You will not be charged the a-la-cart rate.
